The Role:
As a Rehabilitation Case Manager, you’ll:
• Manage your own caseload and diary remotely
• Conduct Initial Needs Assessments & home visits (travel covered)
• Design and coordinate tailored rehabilitation programmes
• Collaborate closely with our expert multidisciplinary team
• Communicate with patients, healthcare providers, insurers & solicitors
• Produce high-quality clinical reports and care plans
About You:
- Qualified Occupational Therapist, Physiotherapist or Nurse
- 3+ years post-graduate clinical experience
- HCPC, NMC or GMC registration
- Current knowledge in complex ortho, neuro, MSK, or psychological injuries (brain injury experience desirable)
- Strong clinical reasoning, report writing, and communication skills
- Confident working independently and collaboratively within an MDT
- Own vehicle and willingness to travel (up to 3 hours)
- UK-based candidates only (we do not offer sponsorship)
Private case management experience not required — we provide full training & mentoring.
