We’re Hiring: Rehabilitation Case Manager (OT / Physio / Nurse)

Remote – Home Based (travel expenses covered)

Full-time, Part-time (min. 3 days / 21 hours) | Associates also considered

South East – Berkshire, Oxfordshire, Wiltshire, Hampshire, Surrey

The Role:

As a Rehabilitation Case Manager, you’ll:

• Manage your own caseload and diary remotely

• Conduct Initial Needs Assessments & home visits (travel covered)

• Design and coordinate tailored rehabilitation programmes

• Collaborate closely with our expert multidisciplinary team

• Communicate with patients, healthcare providers, insurers & solicitors

• Produce high-quality clinical reports and care plans

About You:

  • Qualified Occupational Therapist, Physiotherapist or Nurse
  • 3+ years post-graduate clinical experience
  • HCPC, NMC or GMC registration
  • Current knowledge in complex ortho, neuro, MSK, or psychological injuries (brain injury experience desirable)
  • Strong clinical reasoning, report writing, and communication skills
  • Confident working independently and collaboratively within an MDT
  • Own vehicle and willingness to travel (up to 3 hours)
  • UK-based candidates only (we do not offer sponsorship)

Private case management experience not required — we provide full training & mentoring.
