Job Description: Community Psychiatric Nurse (CPN) Dementia Services Location: Shrewsbury Specialty: Community Nurse Dementia Services Band: Band 6 Reporting To: Team Lead / Service Manager, Dementia Services About the Role An experienced Community Psychiatric Nurse is sought to join a Dementia Services team in Shrewsbury, delivering high-quality, person-centred care to people living with dementia and their families/carers within the community. This is a great opportunity to work as part of a supportive multidisciplinary team, providing assessment, care planning and ongoing support to a caseload of patients. Key Responsibilities Carry out comprehensive assessments of patients with dementia and associated mental health needs Develop, implement and review individualised care plans in the community Provide support, education and advice to patients, families and carers Work collaboratively within a multidisciplinary team (MDT), including psychiatrists, OTs, social workers and GPs Monitor and manage medication, including side effects and concordance Respond to crisis situations and risk assess appropriately Maintain accurate clinical records in line with NMC and trust requirements Support junior staff and contribute to training/development within the team Essential Requirements Registered Nurse (RMN/RNLD or equivalent) with current NMC registration Experience working within community mental health/dementia services Sound knowledge of dementia care, risk assessment and safeguarding Strong communication and organisational skills Full UK driving licence (community role) Right to work in the UK
