Community Psychiatric Nurse (CPN) Dementia Services

Location: Shrewsbury

About the Role
An experienced Community Psychiatric Nurse is sought to join a Dementia Services team in Shrewsbury, delivering high-quality, person-centred care to people living with dementia and their families/carers within the community. This is a great opportunity to work as part of a supportive multidisciplinary team, providing assessment, care planning and ongoing support to a caseload of patients.

Key Responsibilities

  • Carry out comprehensive assessments of patients with dementia and associated mental health needs
  • Develop, implement and review individualised care plans in the community
  • Provide support, education and advice to patients, families and carers
  • Work collaboratively within a multidisciplinary team (MDT), including psychiatrists, OTs, social workers and GPs
  • Monitor and manage medication, including side effects and concordance
  • Respond to crisis situations and risk assess appropriately
  • Maintain accurate clinical records in line with NMC and trust requirements
  • Support junior staff and contribute to training/development within the team

Essential Requirements

  • Registered Nurse (RMN/RNLD or equivalent) with current NMC registration
  • Experience working within community mental health/dementia services
  • Sound knowledge of dementia care, risk assessment and safeguarding
  • Strong communication and organisational skills
  • Full UK driving licence (community role)
  • Right to work in the UK
