Community Psychiatric Nurse (CPN) Dementia Services

About the Role
An experienced Community Psychiatric Nurse is sought to join a Dementia Services team in Shrewsbury, delivering high-quality, person-centred care to people living with dementia and their families/carers within the community. This is a great opportunity to work as part of a supportive multidisciplinary team, providing assessment, care planning and ongoing support to a caseload of patients.
Key Responsibilities
- Carry out comprehensive assessments of patients with dementia and associated mental health needs
- Develop, implement and review individualised care plans in the community
- Provide support, education and advice to patients, families and carers
- Work collaboratively within a multidisciplinary team (MDT), including psychiatrists, OTs, social workers and GPs
- Monitor and manage medication, including side effects and concordance
- Respond to crisis situations and risk assess appropriately
- Maintain accurate clinical records in line with NMC and trust requirements
- Support junior staff and contribute to training/development within the team
Essential Requirements
- Registered Nurse (RMN/RNLD or equivalent) with current NMC registration
- Experience working within community mental health/dementia services
- Sound knowledge of dementia care, risk assessment and safeguarding
- Strong communication and organisational skills
- Full UK driving licence (community role)
- Right to work in the UK
