Base pay range $30.00/hr – $35.00/hr About the Role MSA Home Health and Hospice , part of the Medical Services of America family, is seeking an experienced Part-Time/PRN Home Health Social Worker for our Smyth Co, Washington Co VA location. Provide services according to the physician’s plan of treatment and assist the team in understanding social and emotional factors affecting patient care. Collaborate with the interdisciplinary team to assess and support patients and families. Maintain accurate, timely client care records following policies. Utilize community and family resources to support long-term client goals and adaptation to disabilities. Assist in staff education as requested by the client care manager. Job Requirements Bachelor’s or Master’s degree in social work, sociology, psychology, or rehabilitation counseling from an accredited 4-year college or university. At least one year of social work experience, preferably in home care, hospital, rehabilitation, or mental health settings. Current licensure to practice as a social worker in VA. Valid Driver’s License.
