The Role: What You’ll Be Doing

As a Care Coordinator, you’ll act as the central link between clients, healthcare professionals, and field staff. You’ll ensure care packages are delivered smoothly, efficiently, and safely, while delivering outstanding customer service.

Your responsibilities will include:

• Staff Placement & Booking Management
• Assign the right staff to client care packages
• Respond quickly to booking requests

• Additional responsibilities
• Coordinate and monitor care delivery to ensure quality and continuity of care
• Communicate with clients, families, and care teams to address questions and resolve issues promptly
• Maintain accurate records, schedules, and documentation
