We are looking to recruit Social Workers to support the Newport Hospital Discharge Service. Newport Discharge Service is based in the Royal Gwent Hospital, supporting patients to leave hospital at the earliest opportunity. The team consists of qualified and non-qualified staff, who manage and oversee the requirements to support hospital discharge, and work as part of a multi-disciplinary team approach, to achieve positive outcomes for individuals and carers.
The Home First Service has established itself as instrumental in supporting the discharge pathways and is an exciting opportunity for staff to influence discharge and admission practices across our acute sites and improve the hospital experience for our citizens.
The Social Worker will be key in working with individuals, families/carers, and key stakeholders in assisting, coordinating, and making appropriate arrangements to support discharge from hospital.
