Pay rates: £19.32 - £42.19 per hour.

Are you a motivated District Nurse looking for change? Medacs Healthcare are looking for Community Nurses / District Nurses to join our team to provide care for patients in the community in the Congleton area, via East Cheshire NHS Trust.
Main Duties of the Job
- Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge to home.
- Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.
- Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment.
- Implement / refer on to the relevant services as appropriate.
- Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.

Essential Qualifications Required
