Meanwood Group Practice are seeking a highly motivated and enthusiastic Lead Diabetic and Practice Nurse to join our expanding friendly and dynamic team on a permanent basis. Monday 7.30am - 1.30pm, Thursday and Friday 7.30am-6pm (26 hours per week).

Main duties of the job

The Lead Diabetic and Practice Nurse at Meanwood Group Practice will provide advanced, evidence-based nursing care across the full spectrum of general practice services, while leading and developing high-quality diabetes care particularly in managing type 2 diabetes and preferred experience with insulin management. The post holder will work autonomously within the multidisciplinary team, contributing to service development, quality improvement, and enhanced long-term condition management to meet the needs of the local Leeds population.

About us

Meanwood Group Practice is an ever-expanding practice situated in North Leeds. We provide care for approximately 16,000 patients covering a variety of demographics. We are also an active member of the Central North Leeds Primary Care Network (PCN). We are a friendly, dynamic and supportive team of 11 Partners, 3 Salaried GPs, 1 ACP, 6 practice nurses, 1 HCA and 1 Phlebotomist all supported by our experienced reception and administrative teams. The practice provides specialist services for ENT, Minor Surgery, Gynaecology and Menopause. We are also a well-established GP training practice.

Job responsibilities

Lead Diabetes Role

  • Lead and coordinate diabetes care pathways
  • Manage Type 2 and pre-diabetes
  • Undertake comprehensive annual diabetes reviews
  • Initiate and titrate insulin and injectable therapies (if qualified)
  • Interpret HbA1c, lipid, renal and ACR results
  • Perform diabetic foot assessments
  • Develop personalised care plans
  • Reduce health inequalities within the practice diabetes population
  • Educate and train other team members
  • Be proactive in implementing service improvements for diabetes

Practice Nurse Responsibilities

  • Deliver comprehensive general practice nursing services including chronic disease management (Asthma, COPD, Hypertension, CKD, CVD)
  • Cervical cytology
  • Childhood and adult immunisations
  • Wound care and leg ulcer management
  • Contraception and sexual health services
  • NHS Health Checks
  • Health promotion and lifestyle support
  • Infection prevention and control compliance

Person Specification

  • Qualifications: Qualified nurse registered with the UK Nursing and Midwifery Council
  • Type 2 Diabetes training
  • Chronic Disease Management Training

Experience

We are looking for an experienced practice nurse who specialises in type 2 diabetes management to join our cohesive, supportive and experienced clinical and practice team. Experience with insulin management.
