We are searching for an enthusiastic person to support frail vulnerable adults on their reablement journeys, enabling them to regain independence. You will work with people who have been discharged from hospital back into their own homes across North Northamptonshire, supporting them to achieve their goals and helping with daily living needs.
However please be aware that North Northamptonshire Council cannot employ people on a sponsorship. Social Care knowledge and experience is necessary. Full support and training will be provided.
Shift pattern: alternating weekends and varied shift patterns. Unit base: Haylock House, Kettering Parkway, Kettering Venture Park, Kettering, Northants NN15 6EY.
Our main goal is to find people who can make a difference. A full driving licence with access to a vehicle is essential. Our CQC rating is Good.
What will you be doing?
- You will need to be genuinely concerned about people's quality of life and be enthusiastic and able to think creatively.
- You will be enabling individuals to regain their independence.
- You will be working with the customer, setting and achieving goals in their own homes in the community.
- You may be responding to emergency situations in the community in partnership with health services and Seniors in the Team.
- You will be working with customers supporting their personal care and administering medication and nutritional needs.
- Regularly review customers' progress, monitoring the effectiveness and efficiency of the support plan and supporting Seniors within the office to make adjustments as necessary.
- Maintaining case records, including utilising department electronic record systems.
- Supporting a Supervisor to run the shift.
- Meet and greeting customers in their own homes.

We are looking for people who have a genuine passion to help people to live independently in their homes. Physically fit and in general good health. Ability to be flexible and compassionate and act with honesty and integrity and treat others with dignity and respect. Take pride in work with a non‑judgemental attitude to others. Caring experience required.
