Job Description:

We are seeking a dedicated Social Worker to join the Working Age Adults Team in Milton Keynes.

The role involves carrying out assessments and reviews within the scope of the Care Act 2014, facilitating multidisciplinary work, liaising with key stakeholders, managing complex caseloads, supporting transitions, and implementing and monitoring strengths-based interventions.

The successful candidate will be accountable for their own work and delivering cost-effective interventions within agreed budgets.

The team covers adults with a wide spectrum of needs, including learning disabilities, autism, transitions and school leavers, and personality disorders.

We are co-located with a highly efficient team of health professionals, including nurses, SALTs, and psychologists.
