Provide information, advice, and assessments for Service Users returning from hospital or nursing homes, including facilitating discharge with appropriate equipment.

Obtain funding from agencies like charities, NHS, and Social Services when needed.

Collaborate with architects and Service Users to check plans and ensure optimal outcomes; liaise with agencies like Housing, NHS, and voluntary organizations to maximize information dissemination.

Negotiate with Service Users and agencies on providing adaptations considering budget constraints; verify that adaptations meet assessed needs and provide equipment to optimize their use.

Assess and recommend equipment that can alleviate disability effects.

Stay updated on developments in Occupational Therapy equipment, arrange visits for equipment assessment, and ensure correct purchases.

Maintain knowledge of moving and handling techniques and other relevant developments to train care staff.

Assess Service Users for rehabilitation potential and refer to appropriate agencies.

Maintain Continuing Professional Development and professional portfolio to meet Health Professions Council registration requirements.

Perform additional duties as determined by the Director/Manager within the scope of the role and location within Cheshire East.
