We are seeking a highly skilled and motivated Advanced Clinical Practitioner to join our Urgent Community Response Team in Stoke. The successful candidate will play a crucial role in delivering high-quality urgent care services within the community. As a key member of the team, you will be responsible for making senior clinical decisions, working autonomously, and contributing to the effective functioning of the Urgent Community Response service.
Key Responsibilities:
- Senior Clinical Decision Maker: Utilize advanced clinical skills to assess, diagnose, and treat patients with urgent care needs. Make sound clinical decisions, providing leadership and expertise to the Urgent Community Response Team. Collaborate with other healthcare professionals to ensure seamless patient care.
- Autonomous Working: Work independently and autonomously within the scope of advanced clinical practice. Take the lead in managing complex cases, providing timely and effective interventions.
- Urgent Community Response: Respond promptly to urgent care requests within the community. Conduct home visits and assessments, delivering patient-centered care tailored to individual needs.
- V300 Prescriber: Hold a V300 prescribing qualification, demonstrating the ability to prescribe medications within the advanced practitioner role. Prescribe and review medications in line with professional standards and guidelines.
