Responsibilities
- Work 2-3 days a week in the office and 2-3 evenings a week up to 19:30/20:00 when delivering on site.
- Commit to evenings; flexible hours Monday-Friday.
- Cover sites in Cranford, Hounslow, Feltham, Isleworth; office at Chiswick Town Hall W4 4JN.
- Provide individually tailored diet, nutrition, and behaviour change advice to service users / participants (adults and/or children, young people, and families) to manage weight and improve overall health and wellbeing.
- Deliver evidence-based dietary and behaviour change services via one-to-one and group-based sessions, ensuring operational safety of weight management group programmes.
- Support caseload to implement experiments, track behaviours, utilise tech-enabled support, maximise resources, support each other, explore and use wider community provision.
- Provide medium and high‑need nutritional and behaviour change services for eligible participants, empowering children, families, and adults.
- Deliver virtual or face‑to‑face health coaching group delivery sessions in the community.
Essential Qualifications & Experience
- AFN registered degree in Nutrition or HCPC registered dietetics degree with portfolio evidence of continuous professional development.
- Experience delivering group-based behaviour change interventions to children, families or adults; evidence of supporting vulnerable individuals through a change process.
- Experience of caseload management using a Case Management System.
- Experience lesson planning and nutritional/weight management content creation.
- Experience inputting and processing data with accuracy, meeting targets and deadlines.
- Evidence of continuing professional development to provide current health coaching provision.
- Experience working in a health, community or social care environment; good understanding of maintaining a healthy lifestyle.
- Desirable:
- Health coaching qualification or accredited health coaching skills programme.
- Direct health coaching delivery experience.
- Behaviour change interventions for specific lifestyle issues (e.g., smoking cessation, physical activity, NHS Health Checks, weight management, alcohol reduction).
- Working with individuals with long‑term health conditions.
- Experience in community settings.
- Managing own caseload.
- Supporting people with additional needs.

- Strong understanding of social determinants of health.
- Strong understanding of population-based approaches including segmentation and risk stratification; comprehensive knowledge of behaviour change principles and methods such as COM‑B model and behaviour change wheel.
- Excellent oral and written communication skills with diverse backgrounds.
- Good presentation skills and ability to work with individuals and groups.
- Understanding of confidentiality and safeguarding principles.
- Demonstrate core skills and competencies as set out in best practice standards.
- Experience in dealing with metabolic pathways, nutrients, dietary manipulation, obesity, latest nutritional research.
- Effective internal and external stakeholder engagement and management.
- Strong interpersonal relationships; build, develop and maintain relationships.
- Effective caseload management inclusive of accurate data collection, timely recording, delivery against SLAs and SOPs.
- Proficient with digital technologies supporting behaviour change; manage time independently, meet deadlines.
- Effective safe and sensitive data management per information security standards.
- Excellent Microsoft Office skills and quick learning of new systems and processes.
- Commitment to personal development and training.
- Commitment to flexible working (including evenings and weekends) to serve all residents.
- Commitment to supporting team/contract performance to highest standards, undertaking duties per business requirements.

- Experience delivering health screening services (e.g., NHS Health Check).

- Experience planning, conducting, analysing and reporting on nutrition research.
