The Health and Wellbeing Coach provides personalised support to individuals to empower them take control of their own health and wellbeing with a focus on lifestyle behaviour change, inclusive but not limited to the core areas of: Healthy Eating and Nutrition Smoking Cessation Physical Activity Weight Management Alcohol consumption NHS Health Checks (outreach) As a Health and Wellbeing Coach, you will facilitate people in establishing and attaining health-promoting goals, set by the person to change lifestyle-related behaviours, with the intent of reducing health risks, improving self-management of chronic conditions, and improving quality of life. The Health and Wellbeing Coach is also responsible for delivering face-to-face, telephonic, and digital health coaching support to individuals from a wide range of community and healthcare settings via a combination of one-to-one and group-based support in line with need, location and offer. To provide a health coaching service for eligible participants that supports people to gain and use the knowledge, skills, and confidence to become active participants in their health and wellbeing so that they can reach their self-identified health and wellbeing goals, including, but not limited to: Triage and assessment Agenda setting Lifestyle behaviour change support to (e.g.) stop smoking, move more, achieve a healthy weight/die…
