Please note this is a Temporary role for 6 months with an immediate start. Working to support adults, and their families, who have complex social needs and are more vulnerable as a result of disability, exclusion, or diminished capacity, you’ll maximise their potential to live independently. You will assess social care needs using a person centred and strengths-based approach and contribute to the development of care and support plans. The role will require you to manage a caseload, including assessment of risk through direct work, within a quality assurance process. A focus of the role will be to ensure vulnerable people are safeguarded in line with client’s statutory obligations.

Responsibilities

  • The role will be aligned to one of the neighbourhood teams within Colchester or Tendring and you will be expected to cover all the neighbourhoods within either locality.
  • You will be working closely with Urgent Community Response Service to support their role in delivering admission avoidance across North East Essex.
  • You will be expected to undertake Care Act Assessments and commission appropriate services to meet long term needs where reablement services are not appropriate.
  • You will need to work closely with client's Occupational Therapist hub and have a good understanding of Assistive Technology and be approve assessor trained or have an interest in acquiring this knowledge.
  • You will need to work closely with all the neighbourhood teams within your area as well as Discharge to Assess and the Transfer of Care Hub at Colchester Hospital.
  • The role should support to build and provide a collaborative approach to community services through building on the strengths of the community which will include professionals from non-social care services and care givers such as community pharmacists, carers, housing officers, dementia support teams.
  • You will ensure that those that are not eligible for ASC commissioned services are supported to manage their own social care needs and that prevention is key to the intervention.

Requirements

  • You’ll bring a strong ability to build effective relationships with adults and their families in times of challenge and crisis.
  • You will have the drive and passion to want to make a difference, and a willingness to learn.
  • Ideally you will have some experience of activity-based work with adults in a social care setting and some knowledge of legislative and policy guidance applicable within social care settings.
  • You will ideally have knowledge of the local communities.
