We wish to recruit a Therapy Assistant Practitioner to join our innovative, psychology-led Dementia Wellbeing Service, supporting people living with dementia in community and care home settings, as well as their staff and families. This is an exciting opportunity to join a team that is committed to evidence-based practice, high-quality care and improving people’s lives. You will work as part of a large, supportive multidisciplinary team, with Speech and Language Therapists, Occupational Therapists, Dietitians and Psychologists, with your work mainly guided by a Speech and Language Therapist. There will be opportunities to work closely with Mental Health Nurses, Research and Behaviour Practitioners, Assistant Psychologists, Dementia Practice Development Coaches, Admiral Nurses, Non-Medical Prescribers and Advanced Practitioners.
Main duties of the job:
- Deliver evidence-based interventions for people living with dementia, under the guidance of a Speech and Language Therapist and in collaboration with other AHPs.
- Support individuals on a 1:1 and group basis, using communication, cognitive, functional and wellbeing-focused strategies.
- Support provision of post-diagnostic support for people living with dementia in collaboration with AHPs.
- Support delivery of rehabilitation for people living with dementia in collaboration with AHPs.
- Provide education, coaching and practical support to care home staff, families and carers to build confidence and consistency in dementia care.
- Promote care that is person-centred, meaningful and rights-based, recognising each person’s history, preferences and lived experience.
- Take responsibility for the health, safety and wellbeing of individuals during interventions, working within agreed care plans and risk assessments.
- Prepare and adapt therapy resources and equipment, using IT and digital tools where appropriate.
- Maintain clear and accurate records and contribute to outcome monitoring, service evaluation and quality improvement.
- Work closely with colleagues and partner organisations to ensure coordinated, ethical and effective care.

This is a community role in people’s homes and care homes, delivering evidence-based interventions, education and training after induction—both independently and with clinicians. Qualifications and Knowledge:
- Evidence of Level 3 qualification relevant to workplace OR other equivalent training, qualifications or experience (e.g. Psychology Degree, Dementia Care Coach; Skills to Care).
- Level 4 education OR equivalent experience with completion of relevant Level 4 qualification within a specified timescale after successful appointment.
