Position Overview
The successful candidate will be based within the Integrated Discharge Hub – Hospital Social Work Team, delivering a service to patients in St John’s Hospital to facilitate timely discharge from hospital and support people to remain in the community, where they are medically well but need short‑term support due to a change of circumstances.
Team and Service
The Integrated Discharge Hub is an inter‑agency multi‑disciplinary team made up of hospital social workers, community care coordinators, reablement staff, discharge facilitators, AHP staff and representatives from different services.
Key Responsibilities
- Work proactively as part of a team to identify opportunities for Home First while ensuring each patient receives the appropriate level of support to discharge from hospital.
- Carry out tasks in partnership with service users, family members, colleagues in Social Policy and other professionals and agencies as necessary to meet need.
- Maintain a good understanding of Self‑Directed Support and the importance of completing outcome‑focused assessments.
- Participate in essential training, including in adult support and protection training.
- Travel throughout West Lothian as required.
Essential Qualifications and Skills
- SVQ 3 or HNC in Social Care or equivalent, or relevant experience in a social or health‑care setting (essential).
- Desirable: relevant assessment and care management experience in a social work practice team.
Regulatory Requirements
- This post is considered regulated work with vulnerable children and/or protected adults, under the Protection of Vulnerable Groups (Scotland) Act 2007.
- Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment by West Lothian Council.
- Prior to appointment, obtain PVG Scheme Membership through Disclosure Scotland.
