Seeking a compassionate Physical Therapist Assistant to deliver in-home rehabilitative care. You will assist with implementing PT treatment plans, supporting therapeutic exercises, gait training, and functional mobility to promote patient safety, independence, and quality of life.
Key Responsibilities
- Assist in delivering physical therapy services based on established treatment plans
- Support patient rehabilitation through exercise assistance, gait training, and mobility coaching
- Promote safe movement and independence within the home environment
- Accurately document patient visits and clinical information
- Educate patients and families on exercises, progress, and care strategies
- Collaborate with an interdisciplinary team to ensure coordinated care
Qualifications & Skills
- Ability to provide patient-centered care with empathy and professionalism
- Strong communication skills for education and team collaboration
- Knowledge and experience supporting therapeutic exercises and functional mobility
- Accuracy in documentation and attention to patient safety
