Mass General Brigham Home Care provides post‑acute, medically necessary skilled care in the home to help homebound patients recover from illness, surgery or injury and manage exacerbations of chronic disease. Services include skilled nursing care, physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies.

We are seeking a full‑time, 40‑hour Physical Therapist to join the “Paul Revere” team. Most of your time will be spent working in Dorchester, while the entire team supports the following territories: Mission Hill, Jamaica Plain, Roxbury.

Schedule: Monday through Friday. Anticipated daily volume is 5‑6 visits per day plus chart review, visit preparation, documentation, and care coordination.

Physical therapy services are delivered to adult and geriatric patients in their homes with physical impairments, functional limitations, or disabilities resulting from injury, disease or other causes. Collaborate with patients, families, physicians and allied health professionals to identify needs and coordinate services.

  • Ensure accurate and timely daily documentation.
  • Apply standard treatment procedures.
  • Provide teaching to patients and caregivers.
Qualifications
  • One year of experience in acute care, rehab or homecare as a Physical Therapist.
  • Registered with the Board of Registration in Massachusetts.
  • Excellent verbal, written and electronic communication skills.
  • Computer proficiency and ability to learn new software.
  • Ability to prioritize work and adapt to changing situations.
  • Travel required within Mass General Brigham Healthcare at Home’s geographic area.
