GracePath Healthcare is seeking compassionate and dedicated Live-Out Care Assistants to join our growing team of healthcare professionals. This role is ideal for candidates currently residing in the UK and seeking a sponsorship opportunity through a Tier 2 Skilled Worker Visa, especially for those eligible for switching from a student, dependent, or graduate visa. You will be responsible for delivering high-quality care services to elderly and vulnerable individuals in their own homes, ensuring they live with dignity, safety, and comfort.
Key Responsibilities
- Provide personal care including washing, dressing, grooming, toileting, and continence care.
- Assist with mobility and transfers, including the use of hoists or other aids.
- Support with medication administration and meal preparation.
- Offer companionship and emotional support, fostering trust and comfort.
- Monitor and report changes in clients' health or behaviour to the care manager.
- Maintain accurate and timely records of daily care activities and observations.
- Promote independence while respecting client preferences and privacy.
- Adhere to GracePath Healthcare's safeguarding and confidentiality policies.
- Attend regular training, supervisions, and team meetings to ensure continuous professional development.
Required Skills and Qualifications

Working Hours
- Full-time hours: 35–48 hours per week (Live-out).
- Flexible shifts available, including:
- Morning (7:00 am – 2:00 pm)

- Weekend rotations required.
