Job Description Of Team Leader

• Complete all service user assessment, including risk assessments as part of the assessment process, for new service users.
• As required by the company\’s policies, routinely monitor, review, and update the care plan for the client.
• To participate in a rotating system of being on call in case of emergency.
• Should communicate often with care coordinators regarding hiring, distributing care workers, and developing care & support.
• To keep a close line of communication open with the Care Coordinator and Registered Manager.
• Support all field-based carer workers and ensure care workers in the field are observed, supervised, and mentored.
• When a current care worker reports that they won\’t be able to make to attend to a call, assign a suitable replacement care worker.
• To make sure that all written reports, administration, and data kept in a service user’ homes are accurate and current.
• Make sure all client visits are always covered.
• Conduct new care worker orientation visits with clients.
• Immediately notify the care coordinators of any alterations to a care worker or service user\’s schedule.
