Marton Medical Practice is a friendly and busy General Practice based within Whitegate Health Centre in Blackpool. We are looking to recruit a Practice Nurse who is well organised with excellent general Practice Nursing skills, an interest and experience in management of patients with Long term Conditions. The position is 3 full days per week, Wednesday to Friday with hours to suit on these days. You will be working as a member of our Nursing team to deliver a professional and comprehensive service for our patients with full support from our clinical team. Our GPs are all very approachable, providing an excellent environment for personal development. Salary is dependent on experience.
Main duties of the role include responsibility for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients. Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population. The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
Core responsibilities of the Practice Nurse include:
- Support the Infection Prevention Control (IPC) lead by providing audits, training, support and evidence of compliance to the practice manager.
- Develop, implement and embed an effective care management programme for frail and housebound patients.
- Implement and evaluate individual treatment plans for chronic disease patients.
- Provide wound care to patients.
- Review patient medications to enhance compliance and provide guidance regarding side effects and dosages.
- Request pathology services and process results as required.
- Maintain accurate clinical records ensuring SNOMED CT codes are used effectively.
- Support the team in dealing with clinical emergencies and refer patients presenting with mental health needs appropriately.
- Implement vaccination programmes for adults and children.
- Support the clinical team with safeguarding matters.
In addition to the primary key responsibilities, the Practice Nurse may be requested to:
- Develop and review audit protocols and processes for the practice.
- Participate in local initiatives to enhance service delivery and patient care.
- Support all members of the nursing team, providing guidance when necessary.
- Continually review clinical practices, responding to national policies and initiatives.
- Lead the planning and implementation of changes within the nursing team.
Qualifications:
- Essential: Registered Nurse (Nursing and Midwifery Council).
- Desirable: Mentor or teaching qualification, Post graduate diploma or degree (Chronic Disease Management), Family planning qualification.
Clinical knowledge and skills:
- Essential: Clinical knowledge in women's health, chronic disease management, immunisations, and ability to record accurate clinical notes.
- Desirable: Broad knowledge of clinical governance and public health issues.
Personal Qualities:
- Essential: Polite, confident, flexible, high levels of integrity, sensitive and empathetic.
Experience:
- Essential: Experience of working in a primary care environment and infection prevention and control measures.
- Desirable: Experience of working as a practice nurse or community nurse.
