Are you a passionate and motivated Practice Nurse with an interest in diabetes care and long-term condition management? This is an exciting opportunity for a Practice Nurse with diabetes focus to join our team and make a real difference. You will play a key role in delivering high-quality, evidence-based care across chronic disease management, health promotion, screening, and preventative services. You will be supported by an experienced multidisciplinary team and encouraged to develop your skills, particularly in diabetes care, while contributing to a service that is responsive, patient-focused, and continually improving.

Main duties of the job:

  • Deliver high-quality, evidence-based nursing care
  • Undertake diabetes reviews, monitoring, and patient education
  • Support the management of long-term conditions (e.g. asthma, COPD, hypertension)
  • Immunisations and vaccinations
  • Wound care and dressings
  • Cervical cytology (if trained)
  • Health checks and screening
  • Promote health and wellbeing through lifestyle advice
  • Maintain accurate and contemporaneous clinical records
  • Work collaboratively with GPs and the wider multidisciplinary team
  • Provide high-quality, evidence-based nursing care in line with NICE guidelines
  • Suture and staple removal
  • ECGs and spirometry
  • Undertake new patient health checks and routine screening
  • Carry out reviews, monitoring, and patient education for asthma and COPD
  • Support patients to self-manage their conditions
  • Deliver opportunistic health promotion and lifestyle advice
  • Support national and local health campaigns (e.g. vaccinations, NHS health checks)
  • Work in accordance with infection prevention and control policies
  • Support IPC audits and compliance activities as required
  • Administer medications in accordance with PGDs and scope of practice
  • Provide advice to patients on medication use, side effects, and compliance
  • Request and process pathology tests as appropriate
  • Refer patients to GPs or other healthcare professionals when appropriate
  • Participate in practice meetings and training

Person Specification:

  • Qualifications: Registered Nurse (NMC)
  • Experience in chronic disease management
  • Immunisation and vaccination training
  • Cervical cytology training
  • Experience of working in primary care
  • Experience managing long-term conditions, particularly diabetes
  • Experience working autonomously within scope of practice
  • Experience with audit or quality improvement
  • Experience with elderly or housebound patients
