Job Summary

We have recently completed our transformational £21.9 million redevelopment project, creating a purpose-built hospice with enhanced facilities designed to meet the needs of children and young people with life-limiting conditions for years to come.

Our hospice now provides modern, welcoming spaces for families accessing respite, symptom management and end‑of‑life care.

These improved facilities ensure we can continue delivering personalised, specialist care in an environment specifically designed to support children, young people and their families across Yorkshire.

As we open our teenage wing, we are looking to expand our team of Care Team Nurses.

As a valued member of our award‑winning and vibrant clinical team, you will provide palliative care, alongside planned and emergency short breaks, both within the hospice and in the community.

About Us

Martin House provides free specialist palliative care to babies, children and young people with complex and life‑shortening conditions.

Families from across West, North and East Yorkshire have access to our care 24/7, 365 days a year, including specialist doctors.

Every year we care for more than 550 families, at the hospice, in hospital and in families’ own homes, as well as supporting around 200 bereaved family members.

Responsibilities

• To provide high quality, clinically effective and evidence‑based nursing care and practice in accordance with Martin House policies and standard operating procedures.
• To provide holistic 1 to 1 care, including activities such as play and recreation as well as attending to the child or young person’s physical and emotional needs.
• To act as Nurse in Charge or act as shift lead when required.
• Pro‑actively support the development of Care Team Members ensuring outstanding standards of care are delivered at all times.
• Act as a Practice Assessor for student nurses.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols and best practices.
• Assess, plan, implement and evaluate end‑of‑life care plans to meet the diverse and individual needs of children, young people and families.
• Support children and young people to access community, social and leisure activities.
• To contribute to assigned governance group including participation in audits, collecting and documenting feedback and research as required.
• To pro‑actively keep up to date with clinical competencies and e‑learning as required.
• Develop new clinical skills as appropriate.
• Take accountability for own decision making including the delegation of tasks to others.
• Undertake regular contact calls with assigned families to meet their specific needs.
• To participate in general household activities when required, to support the running of the Hospice.
• To support children and young people in the Hydrotherapy pool as required.
• To plan and deliver Hospice at Home when and where required.
• Undertake personal and professional reflective practice to support the revalidation process.
• To ensure that care routines and practice are documented accurately and are clearly communicated.
• To ensure accurate and contemporaneous record adhering to standards of confidentiality at all times.
• To hold each member of the multi‑disciplinary team in positive regard and to foster a culture of respect and consideration.
• To work closely with the Care Team Leader on each shift and inform them of any issues that need resolving.
• To undertake moving and handling techniques, including the use of hoists, pushing wheelchairs and clamping within vehicles.
• Supporting children and young people in accessing planned community, social and leisure activities.
• Non‑medical prescribers only: To undertake prescribing for children and young people with a range of palliative care needs, within your scope of competence, if authorised to do so.

• In order to carry out Hospice@Home, access to a car for business use and ability to drive Martin House Pool Cars is essential.
• As we work with children and adults at risk this role is subject to an Enhanced DBS check with child and adult barring.

Qualifications

Essential

• Nursing qualification such as children’s nurse, learning disability nurse.
• Active NMC Registration with no restrictions on practice.
• Experience of working in a care setting, preferably with children and young adults.

Desirable

• Experience of working in a palliative care setting.
• Demonstration of nursing children with life‑limiting conditions, including children with special educational needs.

Experience

Essential

• Strong organisational skills to manage multiple priorities effectively while maintaining attention to detail.
• Knowledge of current legislation relating to children and young people including best practice in safeguarding children.
• Proficiency in using electronic health record systems for documentation purpose.
• Ability to document clear and concise details of care routines and practice.
• An understanding of care practice within a palliative care service.
• Evidenced ability to adopt a proactive approach to reporting and resolving health and safety issues.
• Demonstrable commitment to maintaining own professional development.
• Experience of working to recognised clinical/medical standards of care.
• Professional experience of administering medicines in a care setting. Proficiency with a range of IT systems.

Desirable

• Previous experience working with children with complex clinical needs.
