Primary Care Clinical Pharmacist – Market Rasen Surgery, Lincolnshire. We are seeking an enthusiastic and forward‑thinking clinical pharmacist to join our practice network. You will work in a patient‑centred role based in the surgeries, with some hybrid working. This evolving role offers significant scope for development and support for independent prescribing if required.

Main Duties of the Job

  • Telephone and face‑to‑face consultations and reviews with patients
  • Managing repeat prescriptions
  • Responding to patient 'askmygp' queries related to medication
  • Long‑term condition reviews – training will be provided if necessary
  • Synchronising medication
  • Reviewing urgent prescriptions
  • Liaising with hospital pharmacists
  • Providing regular support and feedback to practice on prescribing action plans
  • Updating patient records
  • Ensuring compliance with CQC medicine‑related standards
  • Undertaking risk assessments and ensuring compliance with legislation
  • Conducting audits

About Us

Market Rasen Surgery is a medium‑sized dispensing practice located in the centre of town, on the edge of the Lincolnshire Wolds, and within easy reach of hospitals in Grimsby, Lincoln, Gainsborough and Louth. The practice serves a largely rural community of approximately 10,600 patients over about 200 square miles. We provide general medical services, minor surgery, and a range of other services including physiotherapy, occupational therapy and community midwifery. Our multidisciplinary team includes GPs, ACPs, nurses, a senior nurse for older adults, trained HCAs, and attached nursing staff.

Job Description

Responsible for supporting Market Rasen Surgery in delivering effective medicines management within the practice, identifying areas for improvement, and initiating and managing change. This includes delivery of patient services as determined by practice policy and national guidelines.

Patient Services

  • Repeat prescribing and daily authorisation of repeats
  • Synchronising medication
  • Reviewing discharge letters and A&E attendances
  • Reviewing clinic letters requesting medication changes
  • Managing urgent script requests
  • Completing face‑to‑face reviews with patients who have complex medication regimes if needed
  • Liaising with hospital pharmacists
  • Completing annual medication reviews for patients with long‑term conditions
  • Conducting structured medication reviews
  • Running CQC reports to ensure safe prescribing management, and contributing to QOF‑related prescribing indicators and medicines optimisation audits
  • Reviewing nursing/care/residential home patient medication reviews and de‑prescribing, informed by recognised polypharmacy tools (e.g. STOPP/START)
  • Considering 7‑day scripts for vulnerable patients
  • Face‑to‑face consultation in clinics and as requested by doctors, both in practice and domiciliary

Qualifications

  • GPhC registered

Experience

  • At least 1 year of experience
  • Desirable: Working within a GP practice
