About the role

As Clinical Care Practitioner, you will join our friendly team to proactively support the Nursing staff in managing the day-to-day running of our provision, ensuring the delivery of the highest level of person-centred support to residents and their families and friends. You will be a highly experienced carer holding Level 3 H&SC, with a genuine passion for people and the desire to ensure that those we care for can live their best lives by being dedicated, thoughtful and reliable. What’s the job about?

As a Clinical Care Practitioner, you’ll combine hands‑on care with team leader duties. It’s all about ensuring that every resident receives the very best care while mentoring and supporting your colleagues. Here’s a flavour of what you’ll be doing:

  • Supervising your care team so they’re confident and skilled in every part of their role.
  • Supporting residents with a range of personal care needs, while making sure dignity, respect and privacy are always maintained.
  • Assisting and supporting the Nurse to assess and identify the needs of the residents, and developing complete, comprehensive and person-centred care plans.
  • Supporting the nursing team with all aspects of medication according to the home’s policies and procedures.
