This is a community-based REACT service based in Bradford, aiming to prevent unnecessary hospital admissions for patients with unmet palliative care needs identified in the Emergency Department who can be supported at home with rapid response short-term interventions.

Base pay range

Salary: Marie Curie Clinical Pay Scale, aligned with AFC Band 5 £31,049 – £37,796 pro rata per annum + enhancements

Hours/Contract: 37.5 hours per week (Days, 8am – 8.30pm)

Responsibilities

An exciting opportunity to join the community arm of the Marie Curie REACT project based in Bradford. This role involves working as part of a multidisciplinary team providing high-quality, integrated nursing support to patients discharged from the Emergency Department whose needs can be met by rapid response short-term intervention. You will use evidence-based practice to assess, plan, implement and evaluate patient care, supporting a team of healthcare assistants to ensure appropriate clinical care is delivered. The workload is varied and may require regular re-prioritisation and rapid assessment, offering learning in managing multiple conditions and complications effectively.

You will be committed to continuous improvement and to treating people with respect as individuals. A clearly demonstrable person-centred approach and compassionate communication are essential.

The nursing team requires flexible working; both full-time and part-time arrangements will be considered.
