We are recruiting for a Healthcare Assistant to join our Early Identification Service in Bradford.

Overview

Marie Curie is the UK’s leading end-of-life charity. We are the largest non-NHS provider of end-of-life care in the UK, delivering community nursing and hospice care across the country, and providing information and support on dying, death and bereavement. This role works in Primary Care within partnered GP surgeries in Bradford, using the NHS early tool to identify patients who are likely in the last year of life and not yet identified as requiring palliative care.

Responsibilities

  • Be the initial contact with patients, offering support and the opportunity to complete advance care planning.
  • Signpost to hospice, voluntary or community services as appropriate.
  • Visit patients in their own homes and refer them to relevant services.
  • Work under the line management of senior nurses to provide direct support to patients and their families recently identified with a palliative diagnosis.
  • Arrange follow-up, discuss advance care planning, and signpost to support services.
  • Visit local community hubs to provide awareness, education and information on advance care planning.

Salary

Marie Curie Clinical Pay Scale, aligned with AFC 2 £24,465 pro rata per annum

  • Relevant community experience, providing care and support for people with life-limiting illness or palliative care needs

  • Emotional resilience and the ability to work independently, with ongoing training and support
  • UK driver with access to a car and business insurance
