The predicted salary is between 21150 - 25850 £ per year.
Working as part of our community-based team, you'll play a vital role in helping patients remain comfortable, safe and supported during the night, enabling them to stay in the place they choose to receive their care.
You'll provide high-quality, person-centred care that focuses on dignity, comfort and individual needs at a time when compassionate support matters most.
As a lone worker, you'll be responsible for managing your own workload and responding appropriately to changing patient needs throughout the night.
You'll deliver a range of personal and clinical care activities, including support with hygiene, nutrition, hydration, comfort and observation, while recognising when concerns need to be escalated to senior clinicians within the multidisciplinary team.
This role requires a confident, self-motivated Healthcare Assistant who is comfortable working independently in the community.
You'll have a good understanding of palliative and end of life care, strong communication skills, and the ability to make sound decisions within your scope of practice while providing reassurance and support to patients and their families.
