Join our community team in Dudley as a Healthcare Assistant and help provide personalised, dignified end-of-life care to people in the comfort of their own homes. In this rewarding role, you will work as part of a supportive multidisciplinary team, helping people feel safe, comfortable and respected at a highly important time in their lives. You will provide compassionate, person-centred support while helping to preserve each persons dignity, choice and wellbeing.,
- Support individual care plans, including personal care, skin integrity, nutrition and hydration, mobility, rest and sleep.
- Help maintain a clean, comfortable and safe care environment.
- Observe and respond appropriately to changes in a persons condition.
- Support symptom management in line with the agreed care plan.
- Promptly elevate any concerns or changes in condition to registered healthcare professionals.
- Communicate calmly, sensitively and professionally with patients and those important to them.
- Work collaboratively with the wider multidisciplinary team to deliver compassionate, dignified care.
