Are you an experienced palliative care nurse with a genuine passion for innovation and continuous improvement? Marie Curie invites a highly driven and enthusiastic individual to join our new service at North Middlesex University Hospital. In this role, you will be responsible for actively identifying patients in the Emergency Department who are likely in their last year of life, providing specialist palliative care and facilitating their prompt discharge to appropriate care providers, aligned with patients' preferred place of care.

Responsibilities

  • Lead on clinical assessments and care planning
  • Provide advanced symptom control and communication support
  • Coordinate with community and hospital palliative care services to facilitate rapid discharge home or to the preferred place of care
  • Supervise Senior Healthcare Assistants, mentor emergency department staff, and support team learning
