Ben jij een zelfstandige verzorgende IG en houd je van afwisseling? Dan hebben wij een mooie vacature voor je in een van onze zelforganiserende wijkteams. In dat team heeft ieder een eigen rol en organiseren jullie samen het hele zorgproces voor de cliënt: van intake tot ontslag.
Jouw taken:
- Bieden van persoonlijke verzorging en hulp bij medicatie.
- Toezien op de gezondheid van cliënten en tijdig signaleren van veranderingen.
- Ondersteunen en begeleiden van cliënten in hun dagelijks leven.
- Plannen en evalueren van zorgactiviteiten gezamenlijk met collega's.
- Periode en routes plannen, zodat je met je fiets of auto op pad kunt gaan.
