We are currently recruiting for a Full Time Support Planner position at Manuele Praktijk Open Hart (LHRSP Lincoln Dispersed), a community-based support service for people with complex needs using our 40 units of Dispersed Accommodation across Lincoln City, working Monday‑Friday.
In this role you will use your experience of assessing service user needs and planning individually tailored support for people to help them achieve their goals and review progress and achievements. You will be expected to work effectively within a team, carry your own caseload and support your colleagues.
You will be expected to have a good working knowledge of the needs of people struggling with mental health and substance misuse, housing law, debt support and issues related to their ability to live independently.
