We are looking for an ANP to work at our friendly large GP practice. We offer our patients 15-20 minute consultations and support our ANPs with weekly debriefs with a supportive partner.

Main duties of the job:

  • Conducting comprehensive assessments of patients presenting with a wide range of health issues.
  • Providing timely and effective treatment and management plans.
  • Ensuring accurate documentation of patient consultations and treatment plans.
  • Collaborating closely with doctors, care navigators, and other healthcare professionals within the triage hub.
  • Participating in the continuous improvement of clinical processes and protocols.

You would work amongst a large and vibrant multidisciplinary team including GPs, ANPs, Paramedics, Physiotherapists, Pharmacists, Nurses, HCAs, Phlebotomists, all supported by a superb administration and reception team.

Job responsibilities:

  • Work as part of our multi-disciplinary team alongside our GPs, Paramedics & ANPs to deliver safe and high-quality care.
  • Carry out clinical practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
  • Delivery of care in line with NSF, NICE guidelines and evidence-based care.
  • Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
  • Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
  • Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
  • Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
  • Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
  • Refer patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.

Administrative:

  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Participate in the administrative and professional responsibilities of the practice team.
  • Ensure clinical systems kept up to date, recording and/or amending accurate details.
  • Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator.
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.

General:

  • Maintain your own annual registration with the HCPC and adhere to its Code of Professional Conduct.
  • Promote a professional approach at all times to patients and staff, both in and out of the Practice.
  • Maintain your own continuous CPD and ensure a PDP is kept up to date in line with the practice and nursing team development plans.
  • Participate in and attend practice meetings and in-house training.

Person Specification:

Experience:

  • At least 2 years' experience working as level 6 or higher nurse.
  • Experience working in primary care.
  • Experience of working in a GP practice.

Qualifications:

  • Band 6 qualified nurse.
  • Gained or working towards a Masters degree in advanced clinical practice.

Portfolio and clinical practice:

  • Able to provide a competency portfolio.
  • Basic IT skills.
  • Proficient at using EMIS and ANIMA.
