Our client is looking for a Care Coordinator for their care home based in Durham.
The Role – Full-Time Care Coordinator
Full-time, permanent £26,000 per annum.
As a care coordinator, you are responsible for managing the weekly and day-to-day roster of the care team. Using an in-house software system, you’ll be allocating/matching care plans and visits with available staff. You will play an integral role in the quality of the company’s service delivery.
The role involves:
- Managing rotas
- Liaising with service users, family members, and the council’s brokerage team to match staff to allocate hours
- Assisting clients in the absence of a carer, undertaking care calls in extreme emergencies
- Taking the on-call phone weekly
- Conducting initial assessments, creating care plans and risk assessments for new customers
- Reviewing existing care plans
- Supporting customers and their families
- Supporting and training care workers
- Shadowing new carers into their role
- Conducting spot checks, supervision, and staff appraisals
- Updating staff matrix and ensuring compliance is met
- Dealing with staff requests for annual leave
- Assigning the most appropriate care workers to customers
- Making recommendations to the operational manager based on customer care plans
- Creating a good rapport with potential service users
- Listening to personal needs
- Having a good working knowledge of person-centred care
- Creating strong working relationships with other care professionals including social workers, GPs, pharmacists, and district nurses
- Performing associated admin duties like managing care/client files and producing system-generated invoices and compliance documents
Experience in coordinating rotas or working as a coordinator in a domiciliary care or social care company environment is essential. Good telephone manners and the ability to handle telephone inquiries for service users, the local authority, and carers are also required.
