Job overview

This exciting opportunity presents a chance to work within the Central Crisis Response Team and work closely alongside the newly established Frailty Hospital at Home team working on hospital avoidance and caring for patients within their own home environment. The post holder will be responsible for providing a rapid assessment of patients who have urgent care needs in the community. The team has an experienced multidisciplinary team including Advanced Clinical practitioners. You will be expected to support members of the team, act as an autonomous practitioner and champion the role of Occupational Therapy within urgent care. The ability to problem solve and think on your feet is an integral part of the day-to-day running of a Crisis Team. The purpose of the team is to avoid unnecessary hospital admission and as a team we initiate, assess and treat patients with complex, acute and specialist needs to enable them to remain safe and stable within their own homes. Referrals are received from NWAS, GPs, A&E and community services and as a team we work together with primary care, social and voluntary sectors to collaborate services, support and care for the patient.

Main duties of the job

  1. Perform comprehensive assessment of patients with diverse physical, social & medical presentations to provide a clinical assessment and evidence-based intervention and effective discharge management.
  2. Hold responsibility for complex and non-complex cases and be aware of collective responsibility for the team’s caseload, working without direct supervision.
  3. Undertake all aspects of clinical duties as an autonomous practitioner, within scope of practice.
  4. Play an active role with daily handovers, leading on planning and implementation of therapy.
  5. Refer patients to alternative services as determined by their clinical need.
  6. Work on a shift basis to provide Crisis Response service from 8.30am – 10.00pm across 7 days a week via a rota system.
  7. Undertake evidence-based audit and research projects to further own and team’s clinical practice.

Person specification

Education/Qualifications

  • Graduate Diploma in Occupational Therapy
  • HCPC – Health Professionals Council Registration
  • Evidence of relevant post registration training / CPD
  • Clinical Educators Course

Experience

  • Community experience minimum of 2 years
  • Broad experience as a Band 5/6 core clinical areas.
  • Experience of teaching others, mentoring students and supervising junior staff members
  • Experience of urgent assessments and developing and delivering care plans to meet these needs
  • Knowledge of the workings of community therapy services
  • Involvement with audit and research

Skills

  • Ability to organise, prioritise and delegate
  • Good analytical and problem-solving skills – clinical reasoning skills
  • Awareness of own limitations
