This post offers an exciting opportunity to join a multi-agency service for children with Complex Needs within a co-located setting that promotes partnership and collaboration among children, young people, their families, and professionals. We work closely with the Learning Disability Child Adolescents Mental Health Service and the Children’s Intensive Behaviour Support Service, providing opportunities for joint working.
We are seeking an innovative and motivated Registered Nurse with a qualification in Learning Disabilities and experience in a multi-agency environment to join our Children Learning Disabilities Nursing Team. The successful candidate will work with children and young people with learning disabilities, complex health needs, and challenging behaviors. Strong observational, assessment, and analytical skills are essential, along with experience in community or residential settings.
The applicant should have knowledge and experience with evidence-based assessment tools and an interest in continence, health initiatives, and positive behavior support.
CLDNT offers specialist placements for undergraduate LD nurses, with opportunities to supervise students.
Key Responsibilities – Professional/Clinical
- Provide services for children and young people aged 0–18 with significant learning disabilities.
- Manage an allocated caseload, working independently and flexibly.
- Assess health needs, plan, monitor, evaluate, and review care packages.
- Assess and develop individual positive behavioral support plans for children and young people with challenging behaviors.
- Promote inclusion, raise awareness of healthcare needs, and facilitate access to primary and healthcare services.
- Comply with policies and guidelines within the Central Manchester University Hospitals NHS Foundation Trust.
- Assess health and challenging behavior needs within a defined caseload.
- Develop, implement, and evaluate healthcare and positive behavior support plans.
- Conduct joint assessments with other professionals when appropriate.
- Utilize nursing and specialist assessment tools such as Health Action Planning, Continence Advice, and assessments for epilepsy, chronic health conditions, and health checks.
