The Practice Nurse at Manchester Road Surgery will work as a key member of the clinical team, acting within professional boundaries to provide high-quality care for patients. This includes clinical assessment, screening, diagnosis, treatment, and follow-up, while demonstrating safe clinical decision-making and excellent patient care. The role involves chronic disease management (e.g., asthma, diabetes, COPD, hypertension) and working collaboratively with GPs, healthcare assistants, and other members of the practice team.

Main Duties and Responsibilities

  • Provide routine patient care and support clinical procedures.
  • Undertake cervical cytology and support sexual health and family planning services.
  • Perform spirometry and monitor newly diagnosed patients with chronic conditions.
  • Manage chronic disease care and preventative services for conditions including asthma, COPD, diabetes, hypertension, hypothyroidism, IHD, stroke, and learning disabilities.
  • Provide wound care, phlebotomy, injections, ear syringing, and travel health advice.
  • Participate in opportunistic health screening: BP checks, BMI, urinalysis, glucose monitoring, and smoking cessation support.
  • Undertake home visits and support patients requiring tailored care.
  • Communicate effectively with patients regarding their health and care plans.
  • Support junior clinical staff, including healthcare assistants, trainee nurses, and apprentices.
  • Maintain competence in resuscitation and anaphylaxis management.

Professional

  • Adhere to the Nursing and Midwifery Council (NMC) Code of Conduct and professional practice standards.
  • Maintain professional behaviour and uphold Manchester Road Surgery policies and procedures.
  • Engage in continuing professional development (CPD) and clinical supervision with the Lead Nurse or Practice Manager.
  • Participate in clinical audits, quality improvement initiatives, and evidence-based practice reviews.

Communication

  • Communicate clearly and sensitively with patients, families, and carers.
  • Collaborate effectively with colleagues across the clinical and administrative team to ensure coordinated care.
  • Maintain accurate patient records in line with NMC guidelines and practice protocols.
  • Promote effective internal communication across the practice and with external healthcare partners.

Organisational Responsibilities

  • Complete records, audits, reports, and respond to queries as required.
  • Contribute to protocol development, service improvement, and research projects.
  • Participate in practice meetings and support clinical governance and risk management processes.
  • Contribute to significant event audits (SEAs) and infection control monitoring.

Education and Development

  • Support a positive learning environment for staff and students.
  • Undertake mandatory and statutory training and engage in performance appraisals.
  • Participate in clinical supervision and maintain up-to-date skills and knowledge in line with NMC CPD requirements.

Clinical Governance

  • Comply with practice clinical governance standards, policies, and procedures.
  • Assist in the investigation of clinical incidents and complaints as required.
  • Contribute to maintaining a safe and effective practice environment.

Other Duties

  • Demonstrate flexibility to meet the needs of the service and patients.
  • Maintain patient confidentiality at all times.

  • Staff must comply with organisational policies to ensure patient and practice information is managed legally, securely, efficiently, and effectively.

Records Management

  • All staff are responsible for the management of records, including patient, financial, administrative, and personal records. Records may be in paper, electronic, or other formats, and any uncertainties about correct handling must be discussed with the line manager.
