Join Our Preparing for Adulthood (PfA) Team

Due to recent expansion and investment, the PfA Team has exciting new roles available. The team has recently been reconfigured into three sub-teams within a single Service, aligning with Children’s Services to support seamless transition to adulthood. The teams include:
- Preparing for Adulthood: Living at Home
- Preparing for Adulthood: Leaving Care
- Preparing for Adulthood: Complex
We work directly with young people from age 16 (and from age 14 in schools). Our focus is to support their transition and preparation for adulthood, working with young people who may have Learning Disabilities, Difficulties, Autism, ADHD, physical disabilities, health needs, or needs arising from complex trauma and attachment issues. No two days are the same.
Our approach is person-centred, aiming to prevent or delay the need for long-term services. When necessary, we collaborate with the young person and their support circle to ensure the right support is provided to help them achieve their goals. The team operates across children’s and adults’ services, offering a challenging yet rewarding environment with opportunities to shape future services.
Manchester has recently invested in Person-Centred Planners who will work alongside young people, carers, and social workers to develop tailored transition plans. These planners will also support young people in achieving their individual goals, enabling us to make a meaningful difference in their lives.
