Responsibilities

  • Provide person-centred, high quality support to citizens in their own home or in other appropriate locations, to maintain and develop daily living skills that enable participation in activities and access to community services.
  • Provide practical, personal and intimate care while maintaining high standards of hygiene and health and safety.
  • Work with residents in goal setting as detailed in the support plan to maximise independence and rehabilitation through reablement programmes and emotional support.
  • Contribute to future support planning by monitoring, evaluating and communicating with colleagues, external partners and stakeholders.
  • Identify citizen needs and deliver innovative solutions using local community groups and resources to help residents achieve their goals.
  • Complete documentation and correspondence accurately and within agreed timescales, noting progress and changes affecting health, safety and wellbeing of residents.
  • Work in line with National Care Standards, Care Quality Commission and City Council policies, with safeguarding awareness and participation in any investigations as appropriate.
  • Participate in new initiatives and future changes in service delivery.
  • Support citizens and families, and liaise with stakeholders to help citizens achieve their goals and maximise independence and opportunities.
  • Undertake shift work as required; service operates 8am-10pm, Monday to Sunday, 365 days a year.

Qualifications and Requirements

  • To hold or be willing to undertake and achieve a Level 3 Health and Social Care Diploma (QCF).
  • Casual Car Documentation: designated Casual Car User; if driving, hold a full valid driving licence, provide own car for work, and receive an allowance per mile of business use.
  • DBS (Disclosure and Barring Service) clearance: Enhanced DBS check required; information treated confidentially and in line with DBS Code of Practice.
  • Evidence of conduct in previous employment relating to health, social care, or work with children or vulnerable adults; proof of identity; full employment history.

These vacancies are for the Central and North Reablement Team; transfer to the South Team is not possible.
