This is an exciting opportunity to join Manchester’s Reablement service as a Reablement Team Leader D2A Assessor. As a D2A assessor, you will work within the Manchester Local Care Organisation (MLCO) as a Manchester City Council Employee.

The Reablement Team offers an efficient and consistent approach to support which maximises potential for individuals to achieve reductions in care but also ensures consistency and high standards of service delivery.

As a D2A Reablement assessor, you will undertake home visits to carry out holistic assessments, reviews and observations of customers, in order to determine customer needs. You will take a flexible approach to working, which includes a weekend rota to enable timely assessments and discharges from acute/non-acute healthcare settings.

Working collaboratively with directorate/service colleagues and health service colleagues, you will develop and deliver effective reablement and prevention services and implement a city-wide approach in relation to the provision of community-based services in line with the Care Quality Commission (CQC) standards.

You will work alongside a team of reablement staff that provide direct care to customers in their homes in order to help them regain or maintain their independence and ensure that the capacity of the team is managed effectively, through rota and performance management, in order to ensure demand for reablement services can be met.

You will support the day-to-day running and management of the service if required, to support the wider reablement team and according to senior management decisions.

We Are Looking For An Individual Who Can Demonstrate:

  • Communication Skills: Demonstrates an understanding of the views of others and communicates in a realistic and practical manner using appropriate language and medium, listens attentively to views and issues of others and responds to issues arising.
  • Ability to Plan and Organise: Ability to organise your own time effectively, creating own work schedules, prioritising, preparing in advance and setting realistic timescales for self and others.
  • IT Skills: Ability to use ICT systems to obtain and analyse data and present it effectively through a variety of ICT channels.
  • Problem Solving and Decision Making: Ability to analyse situations, diagnose problems, identify key issues, establish and evaluate alternative courses of action and produce a logical, practical and acceptable solution.
  • People Management: Ability to lead, manage and motivate staff to high levels of performance in order to achieve change and maximise staff potential and contribution to the achievement of identified aims and objectives.
  • Financial Management Skills: Work confidently with financial data and make decisions using appropriate financial information, interpreting trends and accounting for risk.

  • Hold or be willing to undertake and achieve NVQ Level 4 in Health and Social Care within six months of commencing in post.
  • Have experience of working in a Social Care or Health Care Setting with an understanding of relevant Social Care legislation.
  • Be willing to work out of hours/weekends/evenings and bank holidays on a rota basis.
  • Hold a full driving licence and access to a car for business use.
