The role involves travelling city wide to carry out visits to vulnerable and disabled citizens in their own homes and to complete holistic functional assessments, identifying and prescribing equipment and minor adaptations to support the citizen and their carers with activities of daily living. The role further involves ordering and fitting of equipment to promote independence and improve the quality of life of our disabled citizens. The candidate will be required to undertake Trusted Assessor Level 3 training and signpost to appropriate services where identified.
The candidate will require a full clean driving licence and have access to a vehicle in order to carry out their visits. They will be required to work flexibly and at times outside of core hours to meet the demands of the service.

They need to be able to prioritise their workload effectively and have excellent IT and written skills.
