Medical Clearance: Pre-placement health assessment (PPHA)

Casual Car Documentation: The successful candidate will be a designated Casual Car User which means that car journeys may be necessary on an occasional basis, but are not essential to the role. If you do drive, you must always hold a full valid driving licence and provide your own car for use at work, as well as provide all the appropriate documentation. You will receive an allowance per mile of business use.

DBS (Disclosure and Barring Service) Clearance: This post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. We are seeking an Assistant Reablement Team Leader to support the delivery of high-quality, person-centred reablement services. This role will combine hands-on practice with day-to-day operational support, helping to ensure individuals with complex needs are supported to regain independence and achieve positive outcomes within their communities.

You will work closely with the Team Leader to coordinate service delivery, support staff, and maintain high standards of care and performance.

Key Responsibilities

Service Delivery

  • Support the delivery of high-quality reablement services within the neighbourhood
  • Carry out and oversee medication assessments and medication audits
  • Ensure service users receive person-centred, outcome-focused care

Team Support and Supervision

  • Assist the Team Leader with day-to-day coordination of the service
  • Provide guidance, informal supervision, and support to team members
  • Support staff to deliver effective, safe, and responsive care
  • Contribute to induction, training, and development of staff
  • Carry out staff observations and staff medication observations
