We are looking for an enthusiastic, highly motivated Clinical Pharmacist to join our patient focused and friendly practice team. We are a very supportive team committed to providing high quality patient care with a strong ethos on team working and integrated care.

Main duties of the job

  • To work as part of a multi-disciplinary team in a patient-facing role.
  • Providing primary support to general practice staff with regards to prescription and medication queries.
  • Support the established repeat prescription system, medicines reconciliation on transfer of care and systems for safer prescribing.
  • Run chronic disease management clinics.
  • Provide structured medication reviews, quality improvement projects as well as undertaking some aspects of the quality and outcomes framework (QOF) and enhanced services.

Our experienced GPs will support and mentor you. We have an open, learning environment with regular staff meetings, audits and SEA meetings.

Our Surgery:

  • Purpose built surgery set within the beautiful Malvern Hills
  • List size of 12,700
  • 4 GP partner practice with 5 salaried GPs, 1 ACP and 2 GP trainees. They are supported by management team, reception team, paramedic, social prescribers and a practice nursing team.
  • EMIS web clinical system

Job responsibilities

  • Ensure delivery of safe, evidence based, cost effective prescribing within an overall quality and clinical effectiveness framework.
  • Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with community pharmacists and secondary care to ensure patients are supported and receive the medicines they need post discharge.
  • To hold clinics for our patients with chronic diseases or for medication reviews and changes.

Person Specification

  • Degree in pharmacy
  • Professional registration with the General Pharmaceutical Council of Great Britain
  • Evidence of ongoing CPD
  • Member of Royal Pharmaceutical Society
  • Postgraduate diploma in clinical pharmacy or prescribing
  • Disclosure and Barring Service Check
