We have an exciting opportunity for a Care Coordinator to join our domiciliary and extra care services across Ellesmere Port and Chester.
As a Care Coordinator, you'll play a vital role in ensuring the smooth delivery of high-quality, person-centred care. You'll work closely with care staff, the people we support, families and healthcare professionals to ensure services are safe, effective and tailored to individual needs. This is primarily an office-based position with occasional field support when required. You'll work on a structured shift rota covering 7am–3pm and 3pm–11pm shifts, including alternate weekends and on-call duties.

- Plan, organise and manage staff rotas to ensure support visits are delivered efficiently
- Complete assessments and care plans to maintain excellent standards of care
- Support care teams through supervision, appraisals and ongoing guidance
- Provide operational support in the field when required
- Ensure services are delivered safely, effectively and in line with regulatory requirements

Previous experience in care coordination or scheduling, a commitment to delivering outstanding person-centred care, a full UK driving licence and access to your own vehicle, and a Level 3 qualification in Health and Social Care would be advantageous.
