About Our Service:

Our Domiciliary & Extra Care Services provide high-quality, person-centred care to individuals in their own homes across Cheshire West and Chester. We support people to live independently, with dignity, and maintain a high quality of life. Working closely with local authorities and other health professionals, we ensure that the people we support receive the tailored care they need, when they need it.

The Role: What You’ll Actually Be Doing

You’ll play a key role in coordinating care for people in their homes. This means:

  • Managing rotas to ensure our care teams are always in place, ready to help.
  • Supporting staff in the field when needed, and stepping in to cover operational demands.
  • Creating and reviewing care plans to make sure each individual’s needs are met.
  • Leading and guiding your team, offering support that helps them grow professionally.
  • Liaising with people, families, and other professionals to ensure safe, high-quality care is always delivered.

What We’re Looking For:

You’ll need experience in care coordination or scheduling, and ideally, a Level 3 in Health & Social Care. But more than that, we’re looking for someone who is organised, communicates well, and is ready to make decisions that improve lives.
