We have exciting new opportunities available for Care Co-ordinators to join our domiciliary & extra care services in Ellesmere Port & Chester.
Salary: £29,973.84 per annum
Weekly Hours: 37.5 hours per week working on a shift-based rota 7am - 3pm and 3pm-11pm including alternate weekends and additional on-call duties.
Location: Ellesmere Port & Chester - office based with field support in both areas as required, a full UK driving licence and access to your own vehicle.
Our services deliver compassionate, person-centred care to individuals in their own homes across Cheshire West and Chester. Working in close partnership with local authorities and other health and social care professionals, we support people to maintain independence, dignity, and quality of life at home. Our services offer a flexible range of both regulated and non-regulated support, tailored to meet individual needs. Key responsibilities will include:
- Responsibility for rota planning and scheduling to ensure the consistent delivery of support visits. This includes creating, implementing, and regularly reviewing.
- Provide field-based support when required to support operational demands.
- Complete comprehensive assessments & care plans, maintaining high standards of person-centred care.
- Provide effective staff supervision and appraisals, offering guidance and support to care teams to promote best practice and professional development.
- Liaise closely with the people we support, their families, and care staff, while managing the day-to-day operational needs of the service to ensure safe, efficient, and high-quality care delivery.
- Work collaboratively with colleagues and other professionals to ensure service delivery standards are met.
Previous experience in scheduling or care coordination is essential and a level 3 in health & social care would be desirable. The role demands strong organisational and communication skills, with confidence in managing staff rotas, maintaining accurate documentation, and providing effective support to care staff. The post holder must be able to work independently, prioritise workload efficiently, and maintain high standards of service delivery.
