This is an opportunity to join the new Building Independence team within Reablement within the MLCO! The service will offer a broad Reablement and Enablement offer to a diverse cohort of individuals, providing a fully trained and skilled team of team leaders and support workers competent in supporting individuals with specialised needs to make lifelong sustainable changes to achieve their goals. The new integrated provision will support cohorts from mental health, learning disabilities, autism, neurodiversity and support to residential provision. The successful candidates will join a service that prides itself in seeing people achieve their goals and return to independence.
The post holder will be responsible for line managing a team of up to 12 staff, this includes appraisals, performance management and adhering to policies and procedures in line with line management. The post holder will undertake both duty and referral tasks, triaging referrals and completing risk assessments, whilst liaising with partners and stakeholders to ensure that risks are managed, and safeguarding procedures are in place to protect individuals and staff. The post holder will work in a person-centred way to ensure positive outcomes for vulnerable individuals and providing resources and advice to ensure individuals are supported to reach goals agreed. The post holder will be responsible for ensuring the service meets the requirement of CQC policies and procedures and undertake regular audits to ensure accountability and standard of service provision.
The candidate will be able to work on a rota working early shifts and late shifts. The hours of operation for the service will be 7.30am-10pm Monday to Sunday (including Bank Holidays) and will work on an 11-week rota covering these hours to ensure staff are supported. This role will attract a 10% flexibility payment.
