The PCN Services Care Coordinator plays an important role within Maidenhead PCN, providing administrative, operational and coordination support to PCN teams and services. The role supports the delivery of projects, clinics, programmes and initiatives that improve the quality of care and enhance the health and wellbeing of local residents.

Working across St Marks Hospital and local GP practices, the postholder will work closely with GPs, practice teams, social prescribing link workers, health and wellbeing coaches, care coordinators, and other multidisciplinary team members within the PCN to ensure services are delivered effectively and efficiently.

The PCN Services Care Coordinator will provide essential administrative, operational and coordination support across Maidenhead PCN. The postholder will support PCN teams with scheduling, service administration, project delivery and day-to-day operational activities, helping to improve the quality of care and enhance the health and wellbeing of residents across Maidenhead.

Maidenhead Primary Care Network (PCN) is a group of 9 GP Practices working together with a range of local providers to offer more personalised and coordinated health and social care to the 84,000-patient population of Maidenhead, Berkshire.
